If you were born in New Zealand you will need to bring your birth certificate with you as proof of ID.  If born overseas you will need your passport and resident/study/work visa as proof of eligibility to receive funded services in New Zealand.

Check out the enrolment form and eligibility criteria below – you can print this and bring it in with you to save time when enrolling.

You will also be asked to sign a ‘Transfer of Medical Notes’ form to release your notes from your previous doctor.

As an enrolled patient you will be entitled to cheaper doctor consultations, and free nurse visits.
